Personal Use Customers

There are 2 methods for initiating an order:

1. Click here to order on-line.
2. Fill-out and sign an OSU Wireless-Cellular Personal Agreement form (Adobe Acrobat Reader is required to access this form.) The first month of service, the cost for the phone, and applicable taxes will be charged to credit card.
